Fees / Insurances – In Network

I am an in network provider for the following insurances and employee assistance programs (EAP’s) so my rate is based on their contracted rate.  These rates are usually discounted.   If you do not have coverage, my private pay rate is comparable to insurance reimbursements.  Reimbursements through HSA accounts are also possible.

In 2018 I will be revising this list and deciding which ones to keep.  Please talk with me directly about any updates with insurance contracts.
